Caracterización clínica de las crisis asmaticas de manejo hospitalario en pacientes pediatricos de edad igual o mayor a 4 años

Childhood asthma is the most common chronic respiratory disease in children and a strong presence in the country, representing a major impact on public health. It is defined as a chronic inflammation of the airways associated with airway hyperresponsiveness that leads to recurrent episodes of wheezing, breathlessness, chest pain and coughing, particularly at night or early morning. This study's main objective is to characterize the pediatric patient aged greater than or equal to 4 years with acute asthma requiring hospital management and whose emergency treatment at home or in the emergency room, it was not effective enough to reverse the acute . We performed a retrospective investigation, descriptive, non-experimental, clinical record review, in children aged greater than or equal to 4 years admitted for acute asthma in the Pediatric Unit A General Roberto del Río Hospital between June 2009 and July 2011. We reviewed a total of 85 medical records. A 68.2% (58/85) of patients were male and the average age was 7.7 years. Patients had, moderate or severe crises. Physical examination reveals an obstructive symptomatology evident to a greater or lesser degree and all in need of supplemental O2. The 55% had a previous diagnosis of asthma and 31.9% of them had been hospitalized previously for the same cause and the average stay was 3.6 days. About 20% of patients required treatment in Critical Patient Unit, where 18.8% received Noninvasive Mechanical Ventilation. The Respiratory Kinesioterapia is positioned as a key element, having presence in 76.5% of cases. Over 50% of patients have nutritional disorders according to their BMI, falling mainly in the group of risk of obesity and obese. None of the patients admitted, died, due to the great experience in the management of the crisis by the Hospital Roberto del Rio, which has pioneered the use of NIV for the treatment of status asthmaticus difficult to resolve....
