Efecto de intervenciones para reducir errores de medicación en una unidad de emergencia de adultos

The Emergency Department (ED) due to its characteristics is vulnerable to the occurrence of Medication Errors (ME), which could reach to deteriorating health or decrease the effectiveness in treatment of the patient. In 2012 took place in Chile, a study that shows the reality about ME in the ED at Hospital Clínico de la Universidad de Chile (HCUCh). After that, was incorporate a set of interventions to reduce ME, finally this study was conducted as a third part of a global study, in order to measure the effectiveness of interventions. A prospective, observational and randomized study of the drug’s use system (DUS) was developed. Data was collected from Monday to Sunday, 8:00 to 21:00 hours, in the months of August and September of the year 2013. In the overall study (pre and post), 728 patients were admitted, with 364 patients for each phase, a total of 1272 drugs were delivered, 626 (49.2%) of which were evaluated in the pre-intervention phase and 646 (50.8%)in the post-intervention. During the post-intervention phase were found a total of 318 ME reaching 153 patients and 247 drugs, prevalence in patients with at least one ME was 54.9% and prevalence in drugs with at least one ME was 49.2%. The therapeutic group with highest incidence of ME was anti-inflammatory and anti-rheumatic (12.7%), the stage of greatest vulnerability was the preparation (23.6%), followed by administration (21.2%), the prescription (10.7%) and dispensing (0.0%). The handwashing failure was the major cause of ME observed in prescription and administration stages, thereby was calculated and compared the ocurrence of ME in patients and drugs, with and without error of the handwashing failure in both phases of study. Considering this failure, a reduction of 40.5% and 44.8% for patients and drugs, respectively was obtained. Meanwhile without considering the handwashing failure was obtained a reduction of 46.3% and 46.7% for patients and drugs, respectively. This study showed that after implementation of set of interventions can reduce the prevalence of patients and drugs with at least one ME in all stages of SUM considering or not the handwashing failure...

Older people (OP) are the patients with the highest consumption of drugs. This situation exposes them to Drug Related Problems (DRP). The aim of this study was to determine DRP with clinical relevance (DRP-CR) in OP attended in an Internal Medicine Service at a high complexity hospital. This was an observational and prospective study, 100 patients were recruited with a survival time greater than 6 months and a stay in the service > 48 hours. At the entry patients were interviewed to know their basic and pharmacological data. On a daily basis, information was recorded from the electronic file, medical visits and the nursing data. DRPs were classified according to The Pharmaceutical Care Project and severity was categorized according to The National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP). All the coded data were analyzed using descriptive statistics. There was an average of 1.5+1.8 DRP-CR per patient. The indication for a drug was the most prevalent DRP-CR (43.2%). According to severity, DRP-CR of lower severity was the most predominant (60%). Laxatives, antihypertensives and analgesics were the drugs mainly involved in DRP-CR. There are not studies in the country; so further research is needed to study the interventions in the health team to improve the patients’ pharmacotherapeutic outcomes...
