This review we sought to critically analyze the literature related to the practice of exclusive breastfeeding (EB) and the factors influencing their debonding is. Objective: To identify factors that influence the maintenance of EB until 6 months of life through a literature review, considering the context in Chile. Methodology: thematic review with search in PubMed, Lilacs, Medline and Scielo. Outcomes: were selected 13 papers where the main factors affecting the EB were maternal age, maternal education, socioeconomic factors, work, parity, type of delivery, professional health advice, reasons for early weaning and support networks. Discussion and Conclusion: mothers with younger age, lower educational and socioeconomic levels, which had surgical or instrumental deliveries and cultural negative factors are the main opponents of maintaining the EB. Positive family dynamics, the presence of support networks and education for health professionals or others, would be very preponderant factors in the installation and maintenance of breastfeeding....

Evaluación del impacto postoperatorio de un protocolo de intervención nutricional perioperatoria en cirugía colorrectal

Context: The Enhanced Recovery After Surgery Protocol (ERAS Protocol) is based on multiple surgical, anesthetics, kinesics and nutritional interventions applied in a simultaneously and coordinated manner in order to reduce stress and postoperative catabolism, accelerating patient`s recovery and reducing postoperative stay. In the Hospital Clínico de la Universidad de Chile, most of these interventions are applied, excepting for the nutritional interventions. Objective: To assess the impact of an ERAS Protocol with perioperative nutritional interventions on the postoperative stay, following elective open surgery for colon and upper rectum. Design: Prospective non-randomized clinical trial. The design involved two groups: a control group with patients operated on Traditional Protocol and an Intervention Group with patients operated on ERAS Protocol with perioperative nutritional interventions. Surgical, anesthetics, kinesics and nutritional interventions were compared between the two groups using Student´s t-test, Wilcoxon test and Fisher´s exact test. The relationship, effect and magnitude of these interventions, regarding postoperative stay, were determined using a bi-variate, multi-variate logistic regression analysis. The success of the intervention was stablished as: postoperative stay ≤ 5 days Vs Failure: prolonged postoperative stay> 5 days. Patients: 41 patients undergoing elective open surgery for colon and upper rectum. 26 patients in the control group operated during 2011 and 15 patients in the intervention group operated during 2012 and 2013. Results: Compared with the control, the intervention group showed a non-significant decrease of 1.5 days in postoperative stay (p=0.436). By the contrary, a significant reduction in postoperative ileus (p=0.029) was found, without affecting postoperative complications and mortality. There was an adherence to ERAS Protocol by 74% (p <0.001) and the only intervention that had an impact on postoperative stay was early postoperative feeding OR 0,21 [0,04-0,93] p=0,041. Conclusion: The implementation of the ERAS Protocol did not reduced the postoperative stay. The early postoperative feeding was the most important protective factor against the prolonged postoperative stay, without increasing postoperative complications or mortality. As the intervention was uneffective, an increment of the number of patients to confirm the hypothesis, is required for further studies....
