Within the different types of industries, one that is characterized by the inherent risks to its activity is the mining industry, where due to the nature of its operations, complexity of systems and methods, there is a high exposure of workers to high level and characteristic risks of the activity, which generates the possibility of suffering an accident with fatal consequences. That is why the companies have as a source of value initiatives about safety of people, activities that are concerned with preventing accidents through the elimination and treatment of risks and impacts. This thesis has as objective the incorporation of the Human Factor as a key security element in the design of tasks associated with mining. This is why we have a study on the causality of the significant events that occurred in the Chilean mining operations of BHP, as well as the development of a practical tool for the analysis of human error. The proposed methodology incorporates the statistical analysis of data, as well as a qualitative analysis of these, the development of a causality model together with its validation through the analysis of surveys, and finally the development of a guide with the objective of identifying the possible human errors that can be committed in the course of the tasks performed in the operation. Among the data used are the reports of significant events, that is, events with fatality potential (including events with and without injury), which occurred between fiscal years 2012 and 2016 at Minera Escondida Ltda. These allow us, through the use of a binary logistic regression model, added to a focus using a Pareto diagram, to define the factors that contribute to the generation of an event with injury. Through a qualitative analysis it is possible to infer the causal relationship that exists between the different factors involved in an injury, generating a model of causality which has as its central driver the "Perception / Recognition of the Danger". This concept acts as a mediating element between the internal conditions of the person and the actions that cause the error. This model was validated through the use of a survey of 114 first-line workers, obtaining favorable results before various validation criteria. Finally, with the conclusions drawn from the study, we have the development of a guide that allows us to identify, through a structured methodology, the possible errors that can be committed in the different steps of a task, in order to implement before the task, controls that reduce the possibility of an event with injury....
