Eficacia de un programa de seguimiento farmacoterapéutico en la adherencia al tratamiento en pacientes con Diabetes Mellitus Tipo II en Atención Primaria de Salud

Abstract

La incorporación del Químico Farmacéutico al equipo de salud en patologías de riesgo cardiovascular optimiza tratamientos y aumenta la adherencia a la medicación. Debido a esto, se evaluó durante 4 meses la eficacia de un programa de Seguimiento Farmacoterapéutico en la adherencia al tratamiento en pacientes con diabetes mellitus tipo 2 de un centro de salud familiar. El diseño del ensayo clínico aleatorio y controlado incluyó la medición de la adherencia farmacológica mediante los test de Morisky-Green-Levine y de Batalla, y el control metabólico mediante hemoglobina glicosilada y otros parámetros clínicos. En este estudio participaron 9 pacientes en el grupo intervenido (GI) asistiendo a una sesión mensual individual estandarizada, y 11 pacientes en el grupo control (GC) asistiendo a 2 sesiones, una al inicio y otra al fin del estudio. Los pacientes del GI y del GC presentaron diferentes características mórbidas y farmacológicas al inicio del estudio. Al analizar los resultados obtenidos se observó un aumento en la adherencia al tratamiento antidiabético de 45% según Morisky-Green-Levine y 11% en test de Batalla en el grupo intervenido. Por su parte, se registró un aumento de 27% en adherencia según Morisky-Green-Levine, pero una disminución de 10% en test de Batalla en el grupo control. Los parámetros clínicos de ambos grupos no tuvieron diferencias estadísticamente significativas al inicio y fin del estudio dentro del mismo grupo. Por lo tanto, es posible concluir que, en los pacientes intervenidos, la participación del Químico Farmacéutico fue efectiva para aumentar los niveles de adherencia en el tratamiento farmacológico mediante seguimiento farmacoterapéutico y educación sanitaria

The incorporation of Pharmacist into a health team in diseases with cardiovascular risk optimizes treatments and increases adherence to medication. Due to this, the efficacy of a Pharmacotherapy follow-up program, related to the pharmacological adherence in type 2 diabetes mellitus patients in a familiar health center was evaluated. The design of the randomized and controlled clinical trial included the pharmacological adherence measuring through the Morisky-Green-Levine and Batalla tests, and the metabolic control through glycated hemoglobin and other clinical parameters. In this study, 9 patients participated in the intervention group (IG) attending a standardized individual monthly session, and 11 patients in the control group (CG) attending two sessions, one at the beginning and one at the end of the study. IG and CG patients had different morbid and pharmacological characteristics at the beginning of the study. When analyzing the results, an increase of 45% was observed in the adherence to antidiabetic treatment according to Morisky-Green-Levine, and 11% according to Batalla test in the intervention group. On its behalf, an increase in adherence of 27% was observed in Morisky-Green-Levine test, but a decrease of 10% in Batalla test in the control group. The clinical parameters of both groups did not have any significant statistic differences at the beginning and at the end of the study in the same group. Therefore, it is possible to conclude that, in the intervention group, the participation of the Pharmacist was effective for increasing the levels of adherence to pharmacological treatments in Type 2 diabetes mellitus outpatients through pharmacotherapeutic follow-up and health education
