Introduction: Terminal chronic kidney disease has become a global public health problem, due to its progressive increase in incidence and prevalence. As a consequence, multipathological and polymedicated patients appear, and the pharmacological adherence is a frequent and very relevant problem in clinical practice, mainly in patients with chronic treatment and with a high complexity of the therapeutic regimen, such as pharmacotherapy for the control of bone-mineral metabolism (BMM). Objective: To determine the effect of pharmacotherapeutic follow-up on adherence to pharmacological treatment of BMM alterations in patients on chronic hemodialysis. Methodology: A prospective uncontrolled pharmacotherapy follow-up study was conducted evaluating pharmacological adherence to BMM therapy, using three different methodologies: SMAQ, serum levels (phosphorus and paratohormone) and pill count, including a historical control group to compare the data obtained. In addition, the complexity of drug therapy was evaluated and, in case of detecting different drug-related problems (DRP), pharmaceutical interventions were performed, based on scientific literature. To reduce biases and confounding variables, an evaluation of these was performed throught a multivariable logistic regression. Results: The study included 33 patients, who were mainly men (63.6%), with an average age of 54.1 ± 13.3 years and half of them in hemodialysis time of 5 years. The average number of comorbidities was 2.2 ± 1.1, with the most frequent associated pathologies being hypertension and type 2 diabetes mellitus. The initial adherence of phosphate binder, according to SMAQ, serum phsphorous level and count pills was 10 (30.3%), 14 (42.4%) y 11 (33.3%) patients, respectively, observing increase in therapeutic adherence after follow-up and significant differences with respect to historical control. In relation to patients with secondary hyperparathyroidism treatment (n=13), according to the SMAQ test and level of PTH, the adherent patients were 7 and 1, respectively, without observing a significant increase in adherence after follow-up or significant differences in relation to historical control. Among the reasons for non-compliance detected, patients prefers not to take medicines and that they forget to administer it. In the case of secondary hyperparathyroidism, failure to attend HD sessions was considere done of the problema in relation to adherence. On the other hand, 63 DRPs were detected at baseline and 54 DRPs during follow-up, with an average appearance of 1.9 ± 1.2 y 1.6 ± 1.2, respectively, with the main DRPs identified using a unnecessary treatment, use of subtherapeutic doses and untreated medical condition, where the main therapeutic groups involved correspond to proton pump inhibitors, phosphate binders and erythropoiesis stimulating antianemic agents. The complexity of the therapy was analyzed using the MRCI tool and no differences in complexity were observed during the follow-up time. Throughout the study, 141 interventions were performed, aimed at the optimization of pharmacological therapy and patient education, achieving 89.3% acceptance. Conclusion: This study demonstrates that pharmacotherapeutic follow-up increases pharmacological adherence in the treatment of BMM alterations and provides relevant information for conducting multicentric comparative studies that allow the evaluation of patients' behavior after the time of pharmacotherapeutic follow-up...
