Estado funcional y calidad de vida de pacientes operados artroscópicamente de Manguito Rotador

Abstract

La lesión de Manguito rotador es una patología muy frecuente dentro de la población adulta, secundaria a lesiones por sobrecarga o eventos traumáticos únicos. Es por esto que el tratamiento post quirúrgico que debe recibir el paciente es fundamental para que éste logre obtener una funcionalidad adecuada y sin dolor que le permita realizar sus actividades de la vida diaria, laborales y recreacionales. La presente investigación corresponde a un estudio no experimental, descriptivo, transversal, prospectivo. El objetivo fue evaluar el Estado Funcional, a través del “Score de Constant” y de la Versión Breve del test “Disabilities of the arm, shoulder and hand”, y evaluar la Calidad de Vida mediante la “Short-Form Health Survey” segunda versión, de los pacientes operados de manguito rotador por técnica artroscópica en el Instituto Traumatológico de Santiago de Chile, entre los años 2008-2009. El estudio se llevó a cabo en 39 pacientes, 24 mujeres y 15 hombres, con edades entre los 35 y 75 años (media de 58,28 ± 8,84). Para el caso de la Funcionalidad, se distribuyeron los resultados funcionales en 4 categorías: mala, regular, buena y excelente. En el “Score de Constant” la mayoría de los pacientes obtuvieron una buena funcionalidad, mientras que en la Versión Breve del test “Disabilities of the arm, shoulder and hand”, la mayoría obtuvo una funcionalidad excelente. Por otra parte, respecto a la Calidad de Vida, si bien no se encontraron categorías, si se obtuvo que el componente de dicha variable que se vio más afectado fue el Componente de Salud Física, respecto al Componente de Salud Mental. Se concluye que los pacientes post cirugía artroscópica de Manguito Rotador logran un óptimo resultado funcional por los puntajes conseguidos, la mayoría se reintegró a la misma actividad previa a la cirugía. En relación a la calidad de vida, los valores se acercan a un buen nivel, no obstante, el Componente de Salud Física es el más afectado ya que se aleja más de los valores de referencia para la población general Chilena.

Lesions of the rotator cuff are a highly frequent pathology among the elder population. Hence the importance of post-surgery treatment to obtain adequate functionality in securing a proper and painless functionality allowing you to perform activities of daily living, work and recreation. The current investigation is a non-experimental, descriptive and transversal study. The study evaluated thirty-nine patients, 24 female and 15 male, ages 35 to 75 (mean 58.28± 8.84), that had undergone surgery of the rotatory cuff through arthroscopy at Instituto Traumatológico in Chile, between 2008 and 2009. The objective was to evaluate functional status using the “Constant Score” and the Quick version of the test “Disabilities of the arm, shoulder and hand” and to evaluate the quality of life through “Short-Form Health Survey” second version. Functional outcomes were distributed in 4 categories: poor, fair, good and excellent. In the "Constant Score" most patients achieved good function, while in the Quick version of the test "Disabilities of the arm, shoulder and hand", obtained most excellent functionality. As to quality of life, although there were no categories described, if was found that the component of such variable that was more affected was the Physical Health Component, regarding the Mental Health Component. It is concluded that patients that have undergone arthroscopic surgery of the rotatory cuff obtain optimal functional results, as shown by the results obtained and by the fact that most of them went back to the same activities as before surgery. Concerning quality of life, although results are close to a good level, Physical Component is more affected and stays further apart from values of the general Chilean Population.
