Efecto de la rehabilitación vocal basada en la terapia de resistencia en el agua en sujetos diagnosticados con disfonía funcional

Abstract

El propósito de este estudio fue comparar los resultados de 2 protocolos de terapia de rehabilitación vocal de orientación fisiológica: la terapia de resistencia en el agua (TRA) y la terapia de fonación en tubo con el extremo libre en el aire (TFTA), en sujetos diagnosticados con disfonía funcional secundaria a disfonía músculo tensional I. Veinte sujetos con edades entre 16 y 39 años, participaron de este estudio. La mitad de ellos fue asignada aleatoriamente al grupo experimental (TRA) y la otra mitad al grupo control (TFTA). La primera etapa de este estudio consistió en el diagnóstico médico ORL. En la segunda etapa se realizó una evaluación vocal pre-tratamiento, que incluyó auto valoración perceptual, toma de medidas acústicas, aerodinámicas y EGG. En la tercera etapa se llevaron a cabo 8 sesiones de tratamiento. En la cuarta etapa, se efectuó una reevaluación vocal post-tratamiento incluyendo las mismas medidas de la primera. En la quinta etapa, jueces externos evaluaron perceptualmente la cualidad de voz resonante de los sujetos. Los resultados reflejan cambios significativos en ambos grupos para las variables perceptuales en la comparación intragrupo. Lo mismo ocurrió con la presión subglótica y el umbral de presión de fonación. Para las variables acústicas no hubo cambios significativos intragrupo. No hubo cambios significativos intergrupo en ninguna de las variables estudiadas. Se concluye que la TRA y la TFTA son igualmente efectivas para la rehabilitación vocal de los sujetos con diagnóstico de disfonía funcional secundaria a DMT tipo I.

The purpose of this study was to compare the results of two physiologic voice therapy: water resistance therapy (TRA) and phonation in tube with the free end in the air (TFTA), in subjects diagnosed with functional dysphonia secondary to muscle tension dysphonia I. Twenty subjects aged 16 and 39, participated in this study. Half of them were randomly assigned to the experimental group (TRA) and the other half to the control group (TFTA). The first stage of this study consisted of ENT medical diagnosis. In the second stage, a pre-treatment vocal assessment was made, including self-perceptual assessment, acoustic, aerodynamics and EGG measures. Eight sessions of treatment were performed in the third stage. In the fourth step, a post-treatment vocal re-assessment was conducted, including the same measures than the first one. In the fifth stage, external judges evaluated perceptually the resonant voice quality in subjects. The results show significant changes in both groups for perceptual variables in the intragroup comparison. The same happened with subglottic pressure and phonation pressure threshold. For acoustic variables there was no significant intragroup changes. There were no significant changes in any of the intergroup variables studied. Concluding, TRA and TFTA are equally effective for vocal rehabilitation of patients with functional dysphonia secondary diagnosis to DMT type I.
