Test de concordancia scrpit para la evaluación del razonamiento clínico en estudiantes de la carrera de tecnología médica mención oftalmología en la Universidad de Valparaíso

Abstract

El Tecnólogo Médico (TM) es un profesional del área de la salud, experto en técnicas y procedimientos médicos, que durante su formación en las universidades chilenas puede optar a alguna mención o especialización que esté dentro de la malla curricular de su casa de estudio. La universidad de Valparaíso (UV) es una de las instituciones que ofrece la formación profesional de este individuo, y a partir del año 2012 lo hace en el marco de la actualización del modelo educativo hacia una formación curricular basada en competencias. Para la carrera de tecnología médica en la universidad, se menciona dentro del perfil de egreso y como principal competencia la capacidad del profesional en la resolución de problemas, y mediante la ejecución de exámenes y procedimientos contribuir a diagnóstico y tratamiento de patologías. El Tecnólogo Médico mención Oftalmología (TMO) de la Universidad de Valparaíso posee manejo de distintos procedimientos y técnicas en exámenes oftalmológicos, contribuyendo al diagnóstico, tratamiento y rehabilitación de afectaciones oculares. Esta capacidad de razonamiento clínico (RC) no ha tenido evidencia científica o investigaciones que acrediten el desarrollo de la competencia durante la formación de los TMO. Así mismo dentro de los instrumentos de evaluación presentes en la carrera no se evidencia un método que se encuentre validado y estandarizado para la evaluación del razonamiento. Es así como resulta necesario estudiar esta área y evaluar la posibilidad de implementar y validar un instrumento que sea capaz de evaluar el razonamiento clínico que poseen los estudiantes en la mención de oftalmología. El Test de Concordancia Script (TCS), es una herramienta para la evaluación del RC en diversas áreas, entre ellas los profesionales de la salud. Basado en el RC hipotético-deductivo y la teoría del script, sumado a un sistema de constructo riguroso con una revisión y validación realizada por un panel de expertos, el TCS destaca por su capacidad de acercar a los estudiantes a situaciones clínicas reales y la necesidad de un pensamiento reflexivo para responder sus preguntas. Es así como el objetivo de este estudio fue evaluar la adquisición del razonamiento clínico a través del Test de Concordancia Script en estudiantes que cursan el 8vo semestre de la carrera de Tecnología Médica mención Oftalmología en la Universidad de Valparaíso casa central. Se llevó a cabo un estudio de tipo experimental con una fase cuantitativa, en donde se diseñó un test de concordancia script y validó el instrumento para la carrera de tecnología médica y la mención de oftalmología. El test fue aplicado a los estudiantes que cursaban el 8vo semestre de la mención, a través de una prueba inicial de 30 preguntas, para luego llevar a cabo tres sesiones formativas por cada estudiante de manera individual en donde se utilizó metodología “think aloud”, finalmente aplicar una prueba final de 30 preguntas tipo script, cuya validación de contenido se llevó a cabo a través de una tabla de especificaciones. Para evaluar la confiabilidad a través del alfa de Cronbach, se aplicó TCS de igual manera a estudiantes que cursaban el 5to año de la carrera en la mención de oftalmología. Se elaboraron 90 viñetas tipo script en conjunto con un TM mención oftalmología de mayor experiencia clínica y docente en la V región, de las cuales 72 fueron validadas por el panel integrado por 12 tecnólogos médicos de al menos tres años de experiencia clínica. La confiabilidad del instrumento en los alumnos de 5to año, quienes habían aprobado el VIII semestre el año anterior fue de 0,702, mientras que el alfa de Cronbach en el grupo de 4to año, quienes cursaban el VIII semestre y fueron parte de las sesiones formativas fue de 0,703 en la prueba inicial y 0,903 en la prueba final. Se logró diseñar un test de concordancia con validez de contenido y constructo, con un alfa de Cronbach que en cada prueba logró ser mayor a 0,7 al corregir y eliminar ciertas preguntas que afectaban la consistencia interna en el grupo control y grupo intervenido. Entre las razones que influyeron en la confiabilidad se debe tener presente que esta investigación fue la primera experiencia con el script para el investigador, profesores, tecnólogos médicos validadores y estudiantes, además el bajo número de estudiantes de 5to año que decidieron participar y a la vez el número de sesiones formativas programados. Si bien esta investigación se llevó a cabo bajo un contexto social complejo en nuestro país, a lo cual se sumó la crisis sanitaria por la pandemia covid-19 en este mismo periodo, este instrumento puede ser utilizado para complementar la formación de los estudiantes y junto con las otras estrategias de aprendizaje que incluye la carrera podría permitir el desarrollo del razonamiento clínico en el tecnólogo médico mención oftalmología. This capacity for clinical reasoning (CR) has not had scientific evidence or research to prove the development of competence during the formation of OMT. Likewise, within the evaluation instruments present in the career, there is no evidence of a validated and standardized method for evaluating reasoning. This is how it is necessary to study this area and evaluate the possibility of implementing and validating an instrument that is capable of evaluating the clinical reasoning that students have in the mention of ophthalmology. The Script Concordance Test (SCT) is a tool for evaluating clinical reasoning in different areas, including health professionals. Based on the hypothetical-deductive CR and script theory, added to a rigorous construct system with a review and validation carried out by a panel of experts, the TCS stands out for its ability to bring students closer to real clinical situations and the need reflective thought to answer your questions. Thus, the objective of this study was to evaluate the acquisition of clinical reasoning through the Script Concordance Test in students who are in the 8th semester of the Medical Technology mention Ophthalmology at the University of Valparaíso, central house. An experimental study was carried out with a quantitative phase, where a script concordance test was designed and the instrument was validated for the medical technology career and the mention of ophthalmology. The test was applied to the students who were in the 8th semester of the mention, through an initial test of 30 questions, and then carried out three training sessions for each student individually where the “think aloud” methodology was used. finally apply a final test of 30 script-type questions, whose content validation was carried out through a table of specifications. To evaluate the reliability through Cronbach's alpha, TCS was applied in the same way to students who were in the 5th year of the degree in ophthalmology. Ninety script-type vignettes were prepared in conjunction with a TM mention ophthalmology with more clinical and teaching experience in the V region, of which 72 were validated by the panel made up of 12 medical technologists with at least three years of clinical experience. The reliability of the instrument in the 5th year students, who had passed the 8th semester the previous year was 0,702, while the Cronbach's alpha in the 4th year group, who attended the 8th semester and were part of the training sessions was 0,703 in the initial test and 0,903 in the final test. It was possible to design a concordance test with content and construct validity, with a Cronbach's alpha that in each test managed to be greater than 0.7 when correcting and eliminating certain questions that affect internal consistency in the control group and the intervened group. Among the reasons that influenced the reliability, it should be noted that this research was the first experience with the script for the researcher, professors, validating medical technologists and students, in addition to the low number of 5th year students who decided to participate and at the same time the number of training sessions scheduled. Although this research was carried out under a complex social context in our country, to which was added the health crisis caused by the covid-19 pandemic in this same period, this instrument can be used to complement the training of students and together With the other learning strategies that the career includes, it could allow the development of clinical reasoning in medical technologist ophthalmology mention.
