Introduction: Potentially inappropriate medications (PIM) and potentially omitted medications (POM) use is considered a public health problem since it increases the morbidity and mortality of the older adults (OA). For this reason, tools have been developed for the detection of PIM and POM, such as the STOPP & START criteria. Objectives: The aim of this study was to determine the prevalence and factors associated with the use of PIM in a sample of OA hospitalized in the internal medicine service (IMS). Methodology: Analytical, observational cross-sectional study nested in a randomized and pragmatic clinical trial. The data was obtained from the FONIS SA14ID0141 project database, collected between 2015 and 2018. The sample included 613 hospitalized patients aged ≥60 years. The prevalence of PIM was determined according to STOPP 2015 at admission, during hospitalization and at some point, namely, at admission or during hospitalization. While the prevalence of POM was determined only at admission according to START 2015. The risk factors associated with PIM or POM were obtained from sociodemographic, morbid and pharmacotherapeutic characteristics of the OA. Data were analyzed using descriptive and comparative statistics. Results: 54.3% of the OA were women and 59.7% were ≥70 years old. The prevalence of PIM at admission and during hospitalization was 27.8% (n=164) and 30.5% (n=187), respectively, while at some point it was 45.7% (n=280). In contrast, the prevalence of POM was 50.2% (n=308) at admission. Risk factors independently and significantly associated with the use of PIM or POM at admission were anguish/depression (OR: 2.0, 95% CI: 1.3 – 3.0, p<0.01), polypharmacy (OR: 1.62, CI 95%: 1.1 – 2.5, p<0.05), coronary heart disease (OR: 6.5, 95% CI: 2.4-17.3, p<0.001), hyperuricemia (OR: 20.6, 95% CI: 7.0 – 60.6, p<0.001) and comorbidities between 2-3 (OR: 4.9, 95% CI: 2.1 – 11.2, p<0.001), 4-5 (OR: 9.3, 95% CI: 3.9 – 21.9, p<0.001), and ≥6 (OR: 19.7, 95% CI: 7,5 – 51.6, p<0.001). Conclusion: Approximately 1 out of 3 OA used at least one PIM at admission and during hospitalization, while 1 out of 2 used at least 1 PIM at some point in the study. Likewise, 1 out of 2 OA had at least one POM upon admission. Patients with anguish and depression, polypharmacy and multiple comorbidities are at higher risk of having MPI and MPO when entering the IMS than those without these conditions. These results warn about the magnitude of the problem and the need to implement strategies to reduce MPI and MPO in EP, which could be directed at those groups identified as being at higher risk...
