Disfagia en adulto mayor: revisión de la evidencia en el manejo de la disfagia en población geriátrica

Objectives: To conduct a literature review that establishes appropriate procedures to treat swallowing disorders in older adults. To establish recommendations for safe feeding in patients with dysphagia, according to the results of the literature review, and thus enhance the quality of life of people who present a swallowing disorder. Methods: Literature review whose inclusion criteria were clinical trials, randomized controlled clinical trial, clinical trial studies in English or Spanish, year of publication from 2006 to 2015, studies in adult population to 60 years of age or higher. Reviews of electronic databases and reference check through keywords related to the management of dysphagia in geriatric population. Results: 21 studies that met the inclusion criteria were included. The evidence shows that there are several therapeutic approaches in swallowing disorders and that the management depend on various factors such as the type of patient, etiology of dysphagia and prognosis of their disease. All these factors also become increasingly important when it comes to elderly patients. Most studies mentioned that not all therapeutic strategies are recommended for geriatric population because of physiological changes of the orofacial structures in aging, however, there are some therapeutic interventions that might benefit the swallowing process in older adults and this technique are more recommend tan other invasive techniques using non-oral feeding. Conclusions: Evidence supports therapeutic intervention of swallowing disorders to promote oral feeding over non-oral feeding. This review shows several strategies both as compensatory and reactivating that would allow feeding in elderly patients with dysphagia, considering the physiological changes that occur in aging. It is important to recognize which are the most suitable procedures for seniors and how these therapeutic strategies can promote oral feeding and increase the quality of life of older adults....
