This research is focused on the hypothesis about the relationship between two methods that allow to test the state of health of patients who suffer from EDR fracture, after one year of medical treatment. It was carried out an osteomorphological evaluation of the wrist, using radiographic plates and standard measures of angle width and length (radial slope, radial length and dorsal slope) between the bony constituents, and a functional evaluation, using Mayo Wrist Scoring Chart as measurement. Our research considered a group made up of fourty patients who suffer from EDR fracture, orthopedically treated between January and September, in 2002 at Instituto Traumatológico de Santiago. The group was made up of 31 women and 9 men, between 20 and 78 years old, with an average of 54,4. Both the functional and radiological score were turned into categories which classify the patient’s wrist condition as excellent, good, regular and bad. Considering Mayo Wrist Scoring Chart, it was found that 13% of the patients were excellent, 25% good, 39% regular and 23% in bad conditions. According to the radiological score, 3% of the patients were found excellent, 52% good, 39% regular and 23% bad. The final results showed that there is not relationship between the variables, osteomorphological and functional condition, giving evidence about the limitation of radiography as the only post-treatment method of control, confirming its complementary role to the medical evaluation....

Introduction: Associating liver partition and portal vein ligation for staged hepatectomy -ALPPS- is a technique that combines portal ligation with parenchymal partition, increasing the liver remnant volume prior to resection. The biological mechanisms involved in this regenerative response are unknown, although they could be associated with the modulation of AMPK/mTOR activity. The aim of this study was to determine the mechanisms involved in hepatic mass increase triggered by ALPPS in an in-vivo experimental model and in patients, including the study of metabolic signals and AMPK/mTOR pathway role. Materials and methods: Sprague Dawley rats (200-230 g) were assigned to 4 groups: ALPPS model, portal vein ligation (PVL), hepatic partition and sham. The liver regeneration rate, serum levels of hepatic regeneration markers, immunohistochemical indicators of hepatocellular proliferation (Ki-67) and hepatic function parameters at 1, 3 and 7 days after surgery were quantified. The expression and activation of molecules involved in the mTOR pathway was assessed by qRT-PCR and WB; and the rapamycin effects, a selective mTOR inhibitor, were analyzed. In turn, a prospective study of patients undergoing ALPPS referred to Hospital del Salvador between 2013 and 2015 was studied. In a first stage, the right portal branch was ligated and liver transected at the limit of segments II-III; followed by second resective time. The volume changes of remnant liver tissue were analyzed by CT-scan using OsiriX® software. In both operative times, serum samples were obtained, as well as of remnant and deportalized liver tissue, performing immunohistochemical analysis (Ki-67) and expression studies of mTOR pathway using qRT-PCR. It was analyzed perioperative morbidity and survival associated with ALPPS. Results: The rats submitted to ALPPS showed a significantly higher liver remnant mass increase versus controls at 24 and 72 hrs, reaching the same mass as the PVL group on the 7th day, which was associated with a significant increase in Ki-67 (+) nuclei (p<0.05). Liver function was preserved in ALPPS rats, although there was transaminases transient increase at 24 hrs, without differences compared with PVL group (p>0.05). Serum IL-6 levels increased in ALPPS versus all control groups (p<0.05). In ALPPS and PVL groups increased mTOR mRNA levels (p<0.05), and the regenerative response of ALPPS rats was significantly suppressed with rapamycin (p<0.01). 11 patients undergoing ALPPS, mean age 58.8 ± 8 years, were enrolled. Remnant liver volume increased 111.6 ± 63.47% (p<0.05) in 9.1 ± 1.59 days, equivalent to 34.43 ± 17.27 mL/day. Ki-67 nuclear staining was significantly higher in hepatocytes of remnant liver tissue at second stage versus the first (p<0.05), with no differences between samples of deportalized lobes. Between 1st and 2nd stages of ALPPS there was a significant increase in liver regeneration serum markers IL-6, TNF-α, HGF and EGF (p<0.05). The mRNA levels of mTOR, S6K1 and 4E-BP1 increased in liver remnant tissue after ALPPS (p<0.05), while AMPK mRNA levels were not significantly modified (p=0.26). There were no significant changes in deportalized lobes. All cases completed the second resective stage, and complications ≥IIIB were present at 36.3% (4/11) of patients. There was no 90-day mortality after ALPPS. Conclusion: ALPPS induces a significant increase of liver remnant mass in a shorter time compared to animals subjected to portal vein ligation and controls; and to classic techniques of portal vein ligation performed on patients, without impairs overall liver function. The remnant liver volume increment triggered by ALPPS is associated with an increase of liver regeneration and cell proliferation markers, which implies an increase in functional hepatocellular mass. A mechanism involved in this response is the activation of the AMPK/mTOR pathway, which regulates cellular processes associated with biosynthesis of macromolecules and cell growth....
