Optimización de la farmacoterapia en adultos mayores hospitalizados durante la pandemia COVID-19

Introduction: A large number of potentially inappropriate medications (PIMs) and drug-related problems (DRPs) are often present in the pharmacotherapy of older adults; therefore, intervention is needed to ensure medication safety. Aim: To optimize the pharmacotherapy by reducing PIM and resolving DRP of hospitalized older adults during the COVID-19 pandemic. Method: By means of interventions, the current prospective follow-up study consisted in optimizing the pharmacotherapy of older adults aged ≥65. A pharmacy intern collected information from clinical records and interviews with patients and/or their caregivers; and participated daily in clinical rounds. Identification of PIM was performed using Beers 2019 criteria and STOPP/START criteria version 2. Additionally, the present and potential DRPs were classified according to the Minnesota Project and the Aggregated System. The multidisciplinary team and patients with/without their relatives received pharmaceutical interventions in the event of detecting DRP or PIM. Results: The mean age of the 57 participants was 81.5 ± 7.0 years and had a mean of 4.5 ± 2.6 comorbidities. The number of patients with at least one PIM, according to Beers 2019 criteria, decreased from 54.4% at admission to 12.3% at discharge. Meanwhile, according to STOPP criteria version 2, this decreased, respectively, from 45.6% to 24.6%. On the other hand, the prevalence of DRP were 91.2% and 89.5% according to the Aggregated System and to the Minnesota Project, respectively. Moreover, the detected and classified DRPs were 282 and 212, sequentially. From all the interventions made to the multidisciplinary team (n=207), 87.4% were accepted and 82.6% were implemented. At last, the application of these interventions, contributed to the resolution of 71.0% of the DRPs according to the Aggregated System and 71.2% according to the Minnesota Project. Conclusion: The pharmaceutical interventions performed contributed to the reduction of PIM use and resolution of DRP in older adults hospitalized in a geriatric specialty care unit during the COVID-19 pandemic. Future studies with appropriate study designs are needed to determine the specific contribution of the pharmaceutical interventions and their consequences...
