Los Adultos Mayores (AM) son los pacientes con el mayor consumo de fármacos, exponiéndolos a Problemas Relacionados con Medicamentos (PRM).
El objetivo de este estudio fue determinar los PRM de Relevancia Clínica (PRM-RC) en los AM en el Servicio de Medicina Interna (SMI) de un hospital de alta complejidad.
El estudio fue observacional prospectivo, reclutándose 100 AM con una sobrevida mayor a 6 meses y una estadía ≥ 48 horas en el SMI. Al ingreso se les entrevistó para conocer sus datos básicos y farmacológicos. Diariamente se registró información de la ficha electrónica, visitas médicas y de la estación de enfermería. Los PRM se clasificaron según “The Pharmaceutical Care” y la gravedad según The National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP). Los datos codificados se analizaron a través de estadística descriptiva.
El promedio de PRM-RC fue de 1,5 ± 1,8 por paciente. La indicación de un medicamento fue el PRM-RC más prevalente (43,2%). Según severidad, los PRM-RC de gravedad menor fueron los más predominantes (60%). Los medicamentos mayormente implicados en PRM-RC fueron laxantes, antihipertensivos y analgésicos.
Al no existir más estudios en el país, se necesitan investigaciones que intervengan en el equipo de salud para mejorar los resultados farmacoterapéuticos de los pacientes

Older people (OP) are the patients with the highest consumption of drugs. This situation exposes them to Drug Related Problems (DRP). The aim of this study was to determine DRP with clinical relevance (DRP-CR) in OP attended in an Internal Medicine Service at a high complexity hospital.
This was an observational and prospective study, 100 patients were recruited with a survival time greater than 6 months and a stay in the service > 48 hours. At the entry patients were interviewed to know their basic and pharmacological data.
On a daily basis, information was recorded from the electronic file, medical visits and the nursing data. DRPs were classified according to The Pharmaceutical Care Project and severity was categorized according to The National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP). All the coded data were analyzed using descriptive statistics.
There was an average of 1.5+1.8 DRP-CR per patient. The indication for a drug was the most prevalent DRP-CR (43.2%). According to severity, DRP-CR of lower severity was the most predominant (60%). Laxatives, antihypertensives and analgesics were the drugs mainly involved in DRP-CR.
There are not studies in the country; so further research is needed to study the interventions in the health team to improve the patients’ pharmacotherapeutic outcomes
