At birth, the fetus-neonatal transition involves important cardiorespiratory changes, allowing an efficient adaptation of the cardiopulmonary system to maintain the life of the newborn. In the Andean Altiplano, a natural hypobaric hypoxia scenario, this transition is compromised due to the lower availability of oxygen, which can generate pulmonary arterial hypertension of the newborn and right heart failure. However, this cardiopulmonary transition has not been well characterized in the neonatal stage. For this reason, this study aimed to characterize the echocardiographic and hemodynamic changes of the newborn lambs in a period of thirty days in both normoxia and hypobaric hypoxia, which allowed for the first time and non-invasively to determine the changes that characterize the transition from fetal to postnatal life. At birth, the highland specimens showed ventricular dysfunction with respect to those of the lowlands, characterized by a reduction in the ejection fraction (~23%), gradual thickening of the interventricular septum and the wall of the right ventricle (~40%). In addition, the pulmonary artery presented increased resistance indices and the S/D ratio at highland. In summary, this study has allowed to know the cardiopulmonary function and structure of the neonate, opening possibilities of predicting results with other species of veterinary medical interest, establishing new goals in the early cardiological diagnosis....

Introduction: Drug-drug interaction is a common problem that has not been studied enough in Chile. This issue is particularly important in geriatrics as their patients are vulnerable to the action of the drugs, polypharmacy, and other various situations, such as complex treatment regimens and attending different medical specialists. The aim of this study was to identify the frequency and severity of the potential interaction between medications in elderly hospitalized patients. Method: The potential drug-drug interaction was evaluated by a retrospective analysis of drugs from patient samples of the Acute Geriatric Inpatient Unit of the Hospital Clínico de la Universidad de Chile (HCUCH). These patients had received pharmaceutical interventions while they were hospitalized from 2006 to 2009 and the information was kept in previously validated pharmaceutical records. The interactions were characterized according to frequency and severity, namely, contraindicated, serious, moderate, minor and unknown. Each one was identified using two specialized databases (Medscape and Drugs) and the results were analyzed using descriptive statistics. Results: The sample population was constituted by 272 patients; female, 65.8%; mean age, 80.7 ± 7.7 years. Most frequent pathologies were hypertension, 71.7%; type 2 diabetes mellitus, 33.1%; and heart failure, 17.3%. The mean of co-morbidities per patient were 4.1 ± 2.2. Polypharmacy was found in 96.3% patients, using a total of 3.076 drugs in the sample, a mean of 11.3 ± 4.6 drugs per patient and 9.0 ± 6.7 days of hospitalization. While Medscape reported 2,626 potential interactions, with a mean of 9.6 ± 8.7 interactions per patient, Drugs reported 3.023 and a mean of 11.1 ± 10.6 interactions per patient. On the other hand, Medscape described potential interactions to 96.3% (262) of patients, and Drugs, to 94.1% (256). When both databases were compared, 1,422 interactions were in common, and of these, 571 were discordant in the allocation of severity level. The patients exhibiting contraindicated potential interactions were 2.6% using Medscape and 3,3 % according to Drugs. About 65% of the patients received drugs with reported serious interactions. The most frequently interacting drugs were dalteparin, paracetamol and ceftriaxone for Medscape and dalteparin, enalapril, and acetylsalicylic acid for Drugs. Conclusions: About 70% of elderly hospitalized patients in a specialized unit exhibited potential interactions of contraindicated and/or serious severity among the prescribed drugs, mainly in prophylactic treatments involving dalteparin.
