Based on the experience developed with older adults for seven years in a charitable institution, applying the psychosocial and community approach, it was noted the beneficial impact on health and functionality of institutionalized older adults, working in teams with gerontologic approach. Also, the achievement of autonomy, participation and involvement of older people requires the team to share a paradigm of intervention, whose patterns, approaches and strategies, facilitate the achievement of cross-sectional therapeutic targets, focused on the real needs of the older person and his/her family. This systematic retrospective of professional experience experienced by nurses and occupational therapists, aims to raise public and persons associated with long-stay facilities, about the relevance, appropriateness and feasibility of gerontological interventions, which need not involve an economic cost, more While a paradigm shift at all levels. To do so, describe the process undertaken for the implementation of comprehensive and meaningful, using as the formation of teams from different disciplines, that value and involve the elderly and their families in making decisions. The intervention of the Occupational Therapist within a health team is characterized by seeking an optimal occupational performance of a person with occupational needs in the various contexts in which they operate. To achieve this, a professional reasoning must be carried out that culminates with the construction of an occupational diagnosis, which is defined as “the result of the professional's judgment on the response of a person, community or organization to their real performance problems or potentials related to their life processes", and being the fundamental basis from which the Occupational Therapy intervention is planned. To guide the students' learning towards a professional reasoning, that end with the elaboration of the occupational diagnosis, the curriculum of Occupational Therapy of Universidad de los Andes has subjects in fourth year, which are carried out simultaneously with the process of external internships, where students begin the interaction with people with diverse occupational needs. The objective of this study is to know the perception of students and graduates of Occupational Therapy of Universidad de los Andes regarding the process of construction of occupational diagnosis and participation in the subject of Practical Clinical Reasoning II. The methodology used is qualitative, from a phenomenological - interpretative perspective. Two methods of data collection were used; first a questionnaire that contained information from a fictitious case, in which the students had to answer an occupational diagnosis; and second, discussion groups with students after the subject, and with graduates from the same University. The analysis of the data was made TAMS Analyzer® 4.49b5ahEC program. Regarding the results, it is described that, as the students progress in their learning itinerary, they were able to elaborate occupational diagnoses considering the person comprehensively, including the fundamental triad in the elaboration of this (person-environment- occupation), however they do not recognize the process of reflection after the practice as a fundamental factor in this process. Regarding the didactic strategies used in the subject, these are recognized as facilitators for the process of construction of the occupational diagnosis, but the students show difficulty in identifying it, unlike the graduates. Atención farmacéutica en adultos mayores atendidos ambulatoriamente en un hospital universitario : estudio piloto de implementación

To worldwide, the elderly are the faster growing age group respect the other ages. In Chile the epidemiologic behavior is not different and elderly has been reach, according to last census to 11.4% of the entire population. This most longevity situation, brings a more technology and sanitary resources consume, among which are drugs used to control of chronic non-transmissible diseases. The simultaneous use of several medications in the elderly increases the risk to develop other kinds of health problems, either for changes in pharmacokinetics, pharmacodynamics, usage habits, etc. This study was developed in the Geriatric Section of the Hospital Clínico Universidad de Chile, and its objective was to implement and evaluate a system of Pharmaceutical Care to elderly attended ambulatory. For this a system of patient attention was designed considering the available resources of the hospital and keeping the habitual medical attention. The established time to pharmacist’s attention was of 45 minutes per patient, and the evaluations that this professional made were Identification of drug related problems (DRP), quality of life related with health and identification of prescription of potentially inappropriate medication (MPI). Also this plan included the participation of Occupational Therapy interns who evaluated trek and balance using the Tinetti Test, and functionality trough the Test of Measure of Independent functionality. Twenty seven patients were evaluated, their mean age was 82 ± 5 years, perception of quality of life related with health was better in the mental than the physical sphere, all presented walking troubles, and 19% of them use 5 or more medications. The most used drugs used were those with action in the Central Nervous System (36,0%) and analgesics (13,0%). In total 65 DRP were found, the most frequently were the consequence of the use of unnecessary drugs (56,8%) and adverse drug reactions (61,3%). Present work allowed to propose and implement professional’s provision inexistent in the hospital, capable to identify risk situation for the elderly related with medication. The results found just represent what happen in this patients and could not be extrapolated to reality of elderly that are attended in the hospital-, It is necessary to develop future experiences in a representative sample for establishing which are the DRP, MPI and the impact that could have its prevention in elderly health attended in the Hospital Clínico Universidad Chile...
