Introduction: Cleft lip and palate (CLP) is a craniofacial malformation which may result in Velopharyngeal Insufficiency (VPI), product of the soft palate structural alteration. This dysfunction affects the speech of the subjects and can produce Compensatory Articulations (CA) by the unconscious attempt to achieve the velopharyngeal sphincter movement. The CA compromises speech intelligibility because its production omits lip and tongue movements that normally achieve the articulatory points of high pressure allophones. The purpose of this study is to contribute to the speech characterization of subjects with a compromised soft palate determining CA prevalence in a group of patients with VPI. Methodology: The research was composed by 24 children with VPI ranging in age from 3 years to 6 years 11 months, and attendding by GES speech and language therapy unit of Roberto del Río Hospital in Santiago of Chile. The velopharyngeal sphincter functionality was assessed in the minors, in addition to the presence and type of CA, through the assessment pattern of cleft patient of the hospital. Results and Conclusions: In the sample, 45.8% of children show compensatory articulations. From these, 54.5% have Glottal Stop AC type and the rest has a combination of both Glottal stop and Pharyngeal Fricative. Most children with AC have an insufficient velopharyngeal mechanism. Therefore, due to the fact that almost half of the children in the sample have damaged speech intelligibility, it is essential to expand the knowledge of the characteristics and therapeutic approach to these difficulties....

Older adults are patients that present as a main characteristic several comorbidities, which is associated to treatments based on many drugs (polypharmacy), thus patients are exposed to a greater risk of receiving potentially inappropriate drug prescriptions (PIDP). It is also known that in these patients an acute illness can change their functionality. A prospective study was performed, in patients aged 65 or older hospitalized in the geriatric ward (GW) of Hospital Clínico de la Universidad de Chile (HCUCh), between August 2007 and Mayo 2008. The purpose of this study was to characterize the population admitted to the geriatric ward and determine the potential relation between the quality of drug prescription and the functionality of the elderly patients during their hospitalization. A total of 100 patients were admitted to the GW of HCUCh during the study period, and 80 of them fulfilled the selection criterion. Of this study group 60% (48) were females, and the average age of the studied group were 81,8 ± 7,0 years. The percentage of patients that were totally or seriously dependant in basic activities of daily living (BADL) corresponds to 29,4%, and 43(60,6%) improved their performance in BADL during hospitalization. During the hospitalization period the patients received an average of 6,8±2,5 drugs/day, and 21(26,3%) received a PIDP.
